Ebola and Civil War Has Left Sierra Leone Bruised - But Far From Broken

In some ways a politician chatting to a group of children about school and football is unremarkable, but these were Ebola orphans and I was in Sierra Leone.

We sat in the afternoon sun, they were shy and sleepy from the heat but smiling just as any child should. Yet the circumstances that had brought them to where we were, are so far from what any parent would wish.

Challenges face Ebola survivors

In her forties, Maimaïssatou Toure is a mother of six and an Ebola survivor from Forécariah Prefecture, Guinea. Known for her dynamism and courage, this woman devoted one year of her life to caring for children whose parents were infected by Ebola and admitted to the treatment centre in Forécariah.

Her story resembles that of many others who have been infected by the Ebola virus disease after contact with a close relative. Unlike some of her family members who were infected but did not survive, including her son, Maimaïssatou Toure beat Ebola after receiving care at the Conakry Donka-based Ebola treatment centre.

“After two weeks of treatment, I tested negative for Ebola and was authorized to go back home to my children and husband. As I expected, people in my neighbourhood avoided any contact with me. They were curious but no one dared coming close to me. What I went through opened my mind and I understood that Ebola patients need support.’’

Scientists are trying to produce antibody from Ebola virus disease survivors

The antibody, known as mAb114, was effective even when given five days after exposure to Ebola virus, researchers said. Although several clinical trials are in progress, there are currently no licenced therapies against Ebola virus.

Immune cells of Ebola survivors show robust, sustained response

“Our findings counter the idea that Ebola virus infection is immunosuppressive, at least in the patients we were able to study,” Anita McElroy, MD, assistant professor of pediatrics at Emory University School of Medicine, said in a press release. “They also demonstrate the value that supportive care may have in enabling the immune system to fight back against Ebola virus infection.”

McElroy and colleagues examined the T- and B-cells of four Ebola survivors to gauge the immune system’s response to Ebola virus infection. Using the CDC’s biosafety level-4 laboratory space, they determined the frequency of activated immune cells, phenotyped activated CD8 T-cells, and measured the kinetics of each patient’s response.

WHO revises guidance for Ebola survivors who nurse

Although Ebola virus RNA was detected at low levels, more evidence is needed to better understand the duration and infectivity of the virus in breast milk, the guidance stated. Spontaneous galactorrhea also was reported in female survivors up to 9 months after symptom onset. Studies assessing the viruses’ persistence are ongoing.

CDC issues interim guidelines for treatment of Ebola survivors

Special consideration is merited for procedures that create the opportunity for contact with certain bodily fluids and tissues in which Ebola virus may persist, the CDC said. These include:

Traditional healers can play a critical role in post Ebola recovery

In many villages in Liberia, it has long been part of the culture for people to routinely search out the services of witch doctors or traditional healers when they are not feeling well. Patients believe they can get better and more affordable treatment, and with access to a basic health care system limited, a traditional healer is the first stop for most when sick. Traditional procedures can include magic, fasting or dieting, bathing, massage, and surgery. Some traditional healers apply blood to the patient’s skin to cure them of their illness.

Progress and hope for Sierra Leone Ebola Orphans

While the media's interest in Ebola has waned and headlines predicting a worldwide epidemic have fallen short of their prophesies of doom, countries such as Sierra Leone continue to be severely affected and impacted by the virus. Kidsave Sierra Leone in partnership with Village Care Initiative unwaveringly continues to fight to find safe and permanent homes for children orphaned by the ongoing epidemic. Currently we are operating in Freetown, Bo, Kenema, Sembehun, Pujehun, and Zimmi.

Clinical care for survivors of Ebola virus disease

Background

The outbreak of Ebola virus disease (EVD) that began in 2013 in West Africa had by December 2015, resulted in over 28,000 cases. Although estimates of the number of people affected during the outbreak vary, up to 10,000 EVD survivors may require convalescent care. A number of both short- and long-term medical problems have been reported in EVD survivors, including mental health issues for both survivors and other family and community members (1-19). In addition, increasing recognition that Ebola virus may persist in selected body compartments of EVD survivors, most notably in the semen of males, brings awareness of the possibility of reintroduction of the virus in areas where transmission has previously been eliminated. EVD survivors need comprehensive support for the medical and psychosocial challenges they face and also to minimize the risk of continued Ebola virus transmission, especially from sexual transmission. This document provides guidance on providing the necessary care and services for clinical care and virus testing, and should be used to guide the planning and delivery of ongoing health services to people who have recovered from EVD.
